Aging & Long Term Care of Eastern Washington (ALTCEW) has an open procurement process for contracted service providers and vendors that serve clients who are eligible for Health-Related Social Needs (HRSN).
Health-Related Social Needs is a Medicaid Transformation Project initiative designed to allow eligible Medicaid enrollees to receive evidence-based, non-medical services to address an individual’s unmet, adverse social conditions that contribute to poor health outcomes.
Performance Period: through June 30, 2027
A 12-month contract renewal may be available based on funding availability and satisfactory contractor performance.

HRSN Nutrition Supports are designed to reduce food and nutrition barriers that negatively impact health. Through these programs, eligible enrollees can access evidence-based, non-medical services that address unmet social needs and support improved health outcomes.

- Medically-Tailored Meals (Delivered)
- Nutrition Counseling and Education
- Pantry Stocking

Home Accessibility Modifications, Remediation, and Adaptation Devices (HARAD)
ALTCEW is currently accepting applications from contracted service providers and vendors interested in serving Medicaid enrollees who meet the required clinical and social risk criteria.
The HRSN HARAD program is designed to provide physical modifications to a home that are necessary to address the health, welfare, and safety of an individual or to enable them to function with greater independence in their home.
Through these services, eligible enrollees can address identified home-based health and safety risks while supporting their overall health, safety, and well-being within their living environment.
