Health Related Social Needs
Aging & Long Term Care of Eastern Washington (ALTCEW) has an open procurement process for contracted service providers and vendors that serve clients who are eligible for Health Related Social Needs (HRSN).
Health Related Social Needs is a Medicaid Transformation Project initiative designed to allow eligible Medicaid enrollees to receive evidence-based, non – medical services to address an individual’s unmet, adverse social conditions that contribute to poor health outcomes.
Performance Period: through December 31, 2026
A 12-month contract renewal may be available based on funding availability and satisfactory contractor performance.

HRSN Nutrition Supports
HRSN Nutrition Supports are designed to reduce food and nutrition barriers that negatively impact health. Through these programs, eligible enrollees can access evidence-based, non-medical services that address unmet social needs and support improved health outcomes.

Coming Soon – HRSN Home Accessibility, Remediation, and Adaptation Modifications (HARAD)
HRSN HARAD services offer environmental accessibility, remediation, and adaptation modifications to make physical changes to a home. These changes are essential to ensure the health, welfare, and safety of individuals, allowing them to function with greater independence within their homes.
